Bergen New Bridge Medical Center Launches COVID-19 Vaccination Program at Felician University Campus!

Dear Felician Community and Your Families,

We welcome you to the Bergen New Bridge Medical Center COVID-19 Vaccination Program with Felician University. On June 3rd and June 4th we will be vaccinating on site at Felician University, Ray’s Place, Education Commons, 223 Montross Avenue, Rutherford NJ. These dates will be for the J&J vaccine. In order to receive the vaccine, you will need to be registered and scheduled through the links below. Each person must be individually registered using a unique email address.

J&J is a single dose vaccine.

Important Information for getting the COVID-19 Vaccine:

· You must be 18 years or older to receive the J&J vaccine.

· Be sure to wear a medical mask or a cloth mask over your nose and mouth.

· Arrive no more than 5 minutes prior to your designated time so you can be screened at our front entrance.

· Plan to be at the vaccination site for 45 to 60 minutes, including post-vaccine observation.

· Wear loose-fitting shirts or tops to facilitate giving you the shot in your upper deltoid (shoulder muscle).

· Bring Your Driver’s License, School ID, and Insurance Card

Further Instructions for sign up:

· Select the green arrow for “Felician Unviersity Vaccinations with BNBMC (J&J)”as the location.

· Affirm that you are eligible to receive the COVID-19 vaccine (everyone who lives, works, or studies in New Jersey is now eligible to receive the vaccine); select, “Yes”.

· Give your consent to the documents shared when you are ready; you will need to check both boxes, then click, “I agree”.

· If you are currently experiencing symptoms of COVID-19 or are in isolation or quarantine, you are not permitted to get the vaccine at this time. You must be symptom-free to receive the vaccine. Select “yes” or “no” as appropriate.

· Next, answer the questions displayed by selecting “yes” or “no”.

· Enter your email address.

· You must be 18 years of age or older to receive the J&J vaccine ; select “yes” or “no” as appropriate.

· When asked are you “Employed in Bergen County and reside in a different county?” Or “Part of a Bergen New Bridge Medical Center Vaccination Program?” select “Yes”.

· Complete the “Profile Information” under #1: Select “Felician Unviersity Vaccinations with BNBMC (J&J)”; select “6/3/2021” for your preferred vaccination date.

· To the question, “Are you an employee of the County of Bergen?” select, “No”.

· After completing the pre-registration form, click “Save and Continue“, then select your appointment time, and click “Save & Confirm”.

· You will then see your appointment confirmation and be sent both a text and email confirmation as well. Please disregard the messaging to “provide proof of residency or proof of office location in Bergen County” inside the confirmation email.

Before you arrive for your vaccination:

· Be sure to wear a shirt that provides easy access to your upper arm (shoulder muscle).

· Bring your health insurance card/information, and either a driver’s license or government issued ID card.

· Please note that your health insurance will be billed by Bergen New Bridge Medical Center for the cost of administering the vaccine only. The vaccine is free. If you do not have insurance or your insurance does not cover the cost of vaccine administration, you will NOT be responsible for payment.

After your vaccination:

· Take a photo of your vaccination card.

· Place your vaccination card in a safe place with other important documents. Vaccination cards are difficult to replace if lost.
