The New Jersey Legislature mandates that all full-time undergraduate students must have medical insurance that includes hospitalization. All full-time Felician College students are assessed an insurance fee, as part of their student fees, so as to comply with this state law. A full-time student status is 12 credits or more.
Part time undergraduates and graduate students taking six or more credits are eligible to enroll in the plan. Go to http://www.bollingercolleges.com/felician. To enroll you must pay the cost by check or credit card.
Please note that the Student Accident and Sickness Insurance Plan provides limited medical coverage benefits, and does not cover all services. Please make sure you read the plan brochure for details on coverage.
Students who have adequate hospitalization insurance coverage, may waive the student insurance fee by completing a Student Medical Insurance Waiver form which is located online on the Felician website. The waiver deadline for Fall 2012 semester is September 24, 2012. Spring 2013 waivers will be available online after January 2, 2013 and the deadline to complete the waiver is February 12, 2013.
Your insurance plan has a prescription drug benefit. It covers you, after a $20 copay, to a maximum of $300 per policy year. Prescriptions may be obtained through a Caremark, Inc. participating pharmacy. A list of nationwide participating pharmacies are available for review at www.caremark.com or by contacting Caremark customer service at 1-800-391-6443.
The Student Accident and Sickness Insurance Program is managed by Bollinger Insurance Solutions. The plan name is Monumental Life Insurance Company. The preferred provider network is CHN Solutions. Using these providers will reduce your out of pocket costs. All claim forms must be filed within 90 days for reimbursement. Each sickness or injury requires a new claim form to be submitted. Medical statements and pharmacy bills should be attached to the claim form and submitted to:
